Why it matters: Texas says the awards are a first portion of an estimated $1.4 billion in federal Rural Health Transformation funding over five years.
The Texas Health and Human Services Commission (HHSC) plans to award $87,248,124.49 in grants to 129 rural healthcare providers across the state, Gov. Greg Abbott's office announced on Oct. 29, 2026.
The awards come from Rural Texas Strong, the state's CMS-approved plan under the federal Rural Health Transformation Program. The $87.2 million is part of a $281,319,360.67 first-year award HHSC received from the Centers for Medicare & Medicaid Services (CMS), and the state puts the total at an estimated $1.4 billion in federal funding over five years. The plan is 100% federally funded, and federal funding depends on meeting all applicable program requirements, according to the release.
What the money covers. This round advances three of the plan's six initiatives, the governor's office says:
- "Make Rural Texans Healthy Again," which helps rural hospital districts address chronic conditions such as diabetes, cardiovascular disease, chronic respiratory disease and obesity.
- "Next Generation of the Small Town Doctor and Team," which supports recruiting, training and retaining physicians, emergency medical professionals and other health workers.
- "Infrastructure and Capital Investments for Rural Texas," which pays to replace aging medical equipment and improve existing facilities.
What happens next. These are intended awards: funding becomes available after contracts are finalized. HHSC says it will keep announcing intended awards weekly as it reviews applications, and it has posted a list of recipients, per the release.
